Other Important Information

Data Protection

We undertake to protect any information relating to you as an individual and to not disclose that information to any body other than another medical professional without your explicit prior written consent. A copy of the Data Protection Policy is available on request.

Should you have any comments in respect of the Practice or wish to make a complaint, these communications should be addressed to the Practice Manager/Operations Manager.  If you feel that you have not received an adequate response, please ask for a copy of our complaints procedure which details further steps that can be followed.

Access to Health Records

Should you need access to your medical records, please make a written request to the practice who will be happy to make the arrangements for you.

Zero Tolerance

Please note that this Practice operates the NHS zero tolerance policy for violence and abuse of all staff.

GP Net Earnings 2018/2019

All GP practices are required to declare the mean earnings (e.g. average pay) for GPs working to deliver NHS services to patients as part of their organisation.  The average pay for GPs working at the Lydney Practice in the financial year 2018/2019 was £62,566 before tax and national insurance (this was for one full-time and 6 part-time GPs).

Improved Access Initiative

The Forest of Dean surgeries have developed, as a 'cluster', an initiative whereby some additional clinical appointment sessions have been, and will be, made available to patients.  Reception staffs may be able to offer one of these appoints if relevant (which may include appointments in the evenings and at weekends).  Here is a link to the group website: www.forestgpsurgeries.co.uk

National Diabetes Audit

We are taking part in an important national project about people at risk of diabetes and diabetes care/treatment in the NHS.  If you have diabetes, non-diabetic hyperglycaemia, impaired glucose tolerance or pre-diabetes; we will resultantly have to share information about your condition and treatment.  The type of information shared is strictly controlled by law and enforced by strict rules of confidentially and security.  If you do not want your information used, please inform one of our receptionists. For more information, please see the poster and leaflet included on the 'other documents' page.
